Direct Support Professional (CLS)

Job Summary
Provide ongoing guidance and support to individuals with intellectual and other disabilities, focusing on self-care routines, health, safety, household chores, socialization, and overall wellbeing. Ensure active participation in daily lives by maintaining and building natural supports while promoting independence. Assist with medication administration, meal preparation, and transportation for medical appointments, shopping, and recreational outings. Implement and document treatment plans, including residential habilitation goals and communication logs, to track progress toward valued outcomes. Mentor new Direct Support Staff by demonstrating supportive routines and reviewing training manuals. Adhere to overtime, scheduling, and minimum staffing policies, and participate in inter-disciplinary team meetings to discuss individual progress.
Required Qualifications
- Must submit a valid NYS drivers' license
- Maintain a qualified driver status throughout employment
- Must have the ability to communicate and understand written and oral directives/information
- Must be able to lift and transfer individuals (children and/or adults)
- Must successfully complete fingerprint/criminal background check
- Must complete Training on Personal Outcome Measures (POM) prior to completion of evaluations and assessments
