Job Summary
Provide ongoing guidance and support to individuals with intellectual and other disabilities, focusing on self-care routines, health, safety, household chores, and socialization. Assist with medication administration, meal preparation including diet adherence and cooking skills, and transportation to medical appointments or recreational outings. Ensure a safe and clean home by participating in household chores as needed. Implement and document treatment plans, residential habilitation goals, and medical case notes while adhering to agency policies and minimum staffing procedures. Serve as a member of the inter-disciplinary team by attending meetings to discuss individual progress and goal achievements. Mentor new staff by demonstrating supportive routines and reviewing training manuals. Maintain certification and training expectations while ensuring active participation in the community.
Required Qualifications
- Must submit a valid NYS drivers' license
- Maintain a qualified driver status throughout employment
- Must have the ability to communicate and understand written and oral directives/information
- Must be able to lift and transfer individuals (children and/or adults)
- Must successfully complete fingerprint/criminal background check
- Must complete Training on Personal Outcome Measures (POM) prior to completion of evaluations and assessments
