Direct Support Professional (CLS)

Provide ongoing guidance and support to individuals with intellectual and other disabilities, focusing on self-care routines, health, safety, household chores, and socialization. Assist clients in preparing meals, administer medication per policy, and ensure a safe, clean home environment. Implement and document treatment plans, residential habilitation goals, and medical case notes while participating in inter-disciplinary team meetings. Offer transportation for medical appointments, shopping, and recreational outings. Mentor new staff by demonstrating supportive routines and reviewing training manuals. Adhere to overtime, scheduling, and minimum staffing policies to ensure quality daily support. This role requires regular unsupervised physical contact, a valid NYS driver's license, and the ability to lift and transfer individuals. Successful completion of a fingerprint/criminal background check is mandatory, with Personal Outcome Measures (POM) training required prior to assessments.
