Job Summary
Provide ongoing guidance and support to individuals with intellectual and other disabilities, focusing on self-care routines, health, safety, household chores, and socialization. Assist with preparing meals, medication administration, and transportation to medical appointments, shopping, and recreational outings. Ensure a safe and clean home while maintaining certification and training expectations. Participate in inter-disciplinary team meetings to discuss progress and goal achievements. Mentor new staff by demonstrating supportive routines and reviewing training manuals. Implement and document all treatment plans, including residential habilitation goals and medical case notes. Adhere to overtime, scheduling, and minimum staffing policies. This role requires regular unsupervised physical contact, a valid NYS driver's license, and the ability to lift and transfer individuals.
Required Qualifications
- Must submit a valid NYS drivers' license
- Maintain a qualified driver status throughout employment
- Must have the ability to communicate and understand written and oral directives/information
- Must be able to lift and transfer individuals (children and/or adults)
- Must successfully complete fingerprint/criminal background check
- Must complete Training on Personal Outcome Measures (POM) prior to completion of evaluations and assessments
