Job Summary
Guide individuals with intellectual and other disabilities through self-care routines, health, safety, household chores, and socialization to foster independence. Assist with meal preparation, medication administration, and transportation to medical appointments, shopping, and recreational outings. Implement and document treatment plans, including residential habilitation goals and communication logs, while participating in interdisciplinary team meetings. Mentor new staff by reviewing training manuals and demonstrating supportive routines. Maintain a safe and clean home environment and adhere to scheduling and staffing policies. This role requires a valid NYS driver's license, ability to lift and transfer individuals, and successful completion of a criminal background check.
Required Qualifications
- Must submit a valid NYS drivers' license
- Maintain a qualified driver status throughout employment
- Must have the ability to communicate and understand written and oral directives/information
- Must be able to lift and transfer individuals (children and/or adults)
- Must successfully complete fingerprint/criminal background check
- Must complete Training on Personal Outcome Measures (POM) prior to completion of evaluations and assessments
