Job Summary
Provide ongoing guidance to individuals with disabilities in self-care routines, health, safety, and household chores to promote independence. Assist with meal preparation, medication administration, and transportation to medical appointments and community outings. Implement and document treatment plans, including residential habilitation goals and medical case notes, while adhering to agency policies and staffing requirements. Mentor new staff by demonstrating supportive routines and reviewing training manuals. Participate in inter-disciplinary team meetings and house meetings to discuss progress and gather information. Maintain certification and training expectations, ensuring a safe and clean home environment.
Required Qualifications
- Must submit a valid NYS drivers' license
- Maintain a qualified driver status throughout employment
- Must have the ability to communicate and understand written and oral directives/information
- Must be able to lift and transfer individuals (children and/or adults)
- Must successfully complete fingerprint/criminal background check
- Must complete Training on Personal Outcome Measures (POM) prior to completion of evaluations and assessments
