Job Summary
Provide ongoing guidance and support to individuals with intellectual and other disabilities, covering self-care routines, health, safety, household chores, and socialization. Assist with medication administration, meal preparation, and transportation to medical appointments, shopping, and recreational outings. Ensure a safe, clean home while implementing and documenting treatment plans, including residential habilitation goals and communication logs. Mentor new staff by demonstrating supportive routines and reviewing training manuals. Participate in inter-disciplinary team meetings, house meetings, and maintain all certification and training expectations. Requires regular unsupervised physical contact, a valid NYS driver's license, and successful completion of a fingerprint/criminal background check.
Required Qualifications
- Must submit a valid NYS drivers' license
- Maintain a qualified driver status throughout employment
- Must have the ability to communicate and understand written and oral directives/information
- Must be able to lift and transfer individuals (children and/or adults)
- Must successfully complete fingerprint/criminal background check
- Must complete Training on Personal Outcome Measures (POM) prior to completion of evaluations and assessments
