Job Summary
Provide ongoing guidance and support to individuals with disabilities regarding self-care routines, health, safety, household chores, and socialization. Assist with medication administration, meal preparation, and transportation to appointments and recreational outings. Ensure a safe and clean home while implementing and documenting treatment plans, medical case notes, and communication logs. Participate in inter-disciplinary team meetings, house meetings, and mentor new staff by reviewing training manuals. Maintain certification, adhere to staffing policies, and complete required background checks and Personal Outcome Measures training.
Required Qualifications
- Must submit a valid NYS drivers' license
- Maintain a qualified driver status throughout employment
- Must have the ability to communicate and understand written and oral directives/information
- Must be able to lift and transfer individuals (children and/or adults)
- Must successfully complete fingerprint/criminal background check
- Must complete Training on Personal Outcome Measures (POM) prior to completion of evaluations and assessments

- High school diploma or equivalent
