Job Summary
Provide ongoing guidance and support to individuals with intellectual disabilities, ensuring active participation in self-care routines, health, safety, household chores, socialization, and overall wellbeing. Assist with medication administration according to policy, prepare meals by teaching diet adherence and cooking skills, and ensure a safe, clean home. Follow treatment plans, document progress in case notes and communication logs, and implement residential habilitation goals. Offer transportation for medical appointments, shopping, and recreational outings. Mentor new staff by demonstrating supportive routines and reviewing training manuals. Participate in inter-disciplinary team meetings and house meetings to discuss progress and gather ideas. Maintain certification and training expectations while adhering to staffing policies.
Required Qualifications
- Must submit a valid NYS drivers' license
- Maintain a qualified driver status throughout employment
- Must have the ability to communicate and understand written and oral directives/information
- Must be able to lift and transfer individuals (children and/or adults)
- Must successfully complete fingerprint/criminal background check
- Must complete Training on Personal Outcome Measures (POM) prior to completion of evaluations and assessments
