Visit clients in their homes to help them maintain independence and support individualized goals, including personal care tasks like bathing, showering, and ensuring home safety. Assist families in staying in their own communities while building relationships and reporting concerns to the Designated Coordinator. Support community engagement by attending local events. This role requires reliable transportation, a valid driver's license, and current insurance, with flexibility to provide rides when necessary.
