Job Summary
Provide personal care including bathing, toileting, and hygiene support while lifting clients with assistance. Support exercise and mobility, monitor health and wellness, and document client progress toward personal goals. Accompany clients to outings and events, assist with work-skills development, and help navigate environmental hazards. Support challenging behavior when necessary and follow detailed dietary instructions. Coordinate with teams to support clients effectively and assertively advocate for their needs. Work in community homes, residential facilities, or shared day programs with 3-5 or 5-8 clients depending on location.
Required Qualifications

- Ability to assertively communicate and advocate for clients
- Reliable attendance and punctuality
- Ability to lift 35 lbs
- High School diploma or equivalent
- Valid driver's license, reliable personal vehicle, and valid insurance
- Successful completion of a criminal background check, drug screen, and driving record check
