Job Summary
Assists individuals in the Social Rehabilitation Program with daily living skills, personal hygiene, and a range of training activities (functional, educational, pre-vocational, communication, social, mobility, and recreational skills). Coordinates intake with external referral sources and collaborates with Case Management and Therapists. Provides training and care, assigns attendance based on consumer level, observes and reports changes, maintains program records, assists in developing service plans, and supports store operations to develop budgeting and money-management skills. Requires relevant certifications/clearances and a valid PA driver’s license; benefits include medical/dental, retirement, tuition discounts, and career advancement opportunities.
Required Qualifications
- Must successfully complete course requirements for certifications
- Must successfully obtain Criminal History Clearance from the Pennsylvania State Police, Child Abuse Clearance from the Pennsylvania Department of Human Services and FBI Fingerprint Clearances
- Must have a valid Pennsylvania Drivers License and safe driving record
- Must not be excluded from participation in Medicare, Medicaid, or any other federal health care program
