Job Summary
Provide 1:1 supervision of assigned individuals by ensuring implementation and recording of habilitation and clinical service plans. Support residents with self-care routines including feeding, showering, dressing, and diapering, while assisting with household chores to maintain a safe and clean home. Follow all treatment plans utilizing adaptive equipment, prepare meals focusing on diet adherence and cooking skills, and accompany residents on medical appointments and outings using safe driving practices. Document all events, activities, and progress via Medisked or paper logs, report incidents to the Justice Center as a mandated reporter, and serve as a member of the inter-disciplinary team. Mentor new staff by demonstrating supportive routines and reviewing training manuals. Arrive at the start of scheduled shifts adhering to agency policies regarding call offs and overtime.
Required Qualifications
- valid New York State Driver's License that is also qualified in accordance with agency's insurance standards and maintained throughout employment
- successful completion and maintenance of SCIP and CPR status
- the ability to communicate and understand written and oral directives/information
- an excellent team player with a positive attitude
- regular and substantial unsupervised or unrestricted physical contact with people receiving services
- successful completion of fingerprint/criminal background check
