Job Summary

Provide all aspects of care to program participants to enable independence and productivity, including bathing, grooming, toileting, dressing, feeding, transferring, and ambulation. Administer medications and escort individuals to medical and psychiatric appointments while maintaining confidentiality. Operate agency vans safely and coordinate emergency evacuations, fire drills, and crisis interventions. Document progress in electronic systems, complete daily checklists, incident reports, and medication administration records. Collaborate with the management team to review goal progress, update program plans, and assist with community resource research. Escort participants to evening and weekend recreational activities and moderate physical events.

Required Qualifications

  • High School Diploma or equivalency diploma
  • Fluent in English for client communication
  • Valid New York State Driver's License
  • At least 2 years of driving experience
  • Comfort operating a large vehicle
  • Proficiency in AMAP
  • Proficiency in SCIP
  • CPR certification
  • First Aid certification
  • Ability to pass exams for AMAP, SCIP, CPR and First Aid courses
  • Ability to climb and descend fire escapes
  • Ability to assist others to climb and descend fire escapes
  • Ability to perform first aid for people served
  • Certified AMAP status
  • Ability to enter person served data documentation in the electronic data system
  • Ability to communicate verbally and in writing
  • Ability to attend scheduled staff meetings
  • Ability to attend mandatory in-service training sessions
  • Ability to attend assigned annual meetings
  • Ability to perform ADL tasks including showering, shaving, toileting, hand washing, mouth care, diaper changing, ambulation, G-Tube, feeding, and transferring using Hoyer lift
  • Ability to escort people served on appointments
  • Ability to plan and follow through on recreational activities
  • Ability to participate in moderate physical activities
  • Ability to perform techniques required in AMAP, SCIP, CPR and First Aid

Desired Qualifications

  • Experience supporting adults with intellectual/developmental disabilities or behavioral health needs
  • Some knowledge of computers
