Job Summary
Monitor, implement, and document individual personal goals and behavior programs while assisting clients with self-care, hygiene, meal preparation, housekeeping, socialization, and recreation. Participate in developing individual Service Plans through daily logs, programming suggestions, and staffings, and provide transportation for community integration. Collect, record, and file data as required. Maintain regular attendance and demonstrate professional behavior with integrity toward clients, families, and colleagues. Full-time Monday through Friday schedule; valid driver's license and reliable transportation required.
Required Qualifications
- High school diploma or equivalent
- Background check clearance
- CPR and First Aid basics
- Must be able to pass the TABE test
- Must be 18 years of age or older
- Must be available to work all shifts, including overnights and weekends
- Demonstrated ability to communicate in a clear, comprehensible manner, both verbally and in writing
- Valid driver's license, in good standing for the state of residency
- Access to reliable transportation
- Valid IL statutory minimum liability insurance coverage, bodily injury and property damage
