Job Summary
Coordinate client decision-making, productivity, and community inclusion while promoting independence in daily activities like meals, grooming, and household maintenance. Implement individual training programs, oversee client activities and environments to ensure health and safety needs are met, and address challenging behaviors proactively. Provide direct support for at least 60% of logged time, transport clients to appointments and community activities, and maintain positive communications with supervisors, families, and community partners. Document activity records, reports, and evaluations accurately; prepare staffing schedules; and complete required assessments and monthly reports under case management direction. Participate in the on-call rotation as needed and monitor client and employee activities. Travel throughout Tulare and Kings Counties at least 75% of the time to fulfill service delivery requirements.
Required Qualifications
- 18 years of age or older
- High School Diploma or Equivalent
- Bachelor's degree or 3 years equivalent experience and/or educational equivalent in working with individuals with disabilities
- Aptitude for confidentiality, organizational skills and good interpersonal skills
- Certification in an approved First Aid and CPR class; or successful completion within 45 days of the date of hire
- Insured, registered vehicle in safe working condition
- Proof of current CA driver's license
- Mobile smart device (i.e. cell phone/tablet) with reliable internet connectivity and data
- Ability to travel within and throughout the counties in which services are provided a minimum of 75% of the time
- Successful completion of pre-employment drug screen and background check
- Acceptable DMV record
