Job Summary
Provide assistance with daily activities including socialization, personal care, housekeeping, recreation, transportation, and community integration for individuals with intellectual and developmental disabilities. Accurately document progress and activity while reviewing records to stay current on service plan changes. Deliver clinical expertise in specialized populations such as brain injury or emotional/behavioral disabilities, and accompany individuals to medical appointments to relay orders and information to providers. Travel between sites and individual homes to support therapeutic and behavioral plans aimed at fostering independence.
Required Qualifications
- Must be 18 years of age
- Valid driver's license in good standing
- Strong attention to detail

Desired Qualifications
- High school diploma or equivalent preferred
- Six months of experience in human services preferred
