Direct Service Provider PT works with consumers daily in community, home, residential, or other treatment settings to provide oversight and support. Responsibilities include assisting with self-care, independence, mobility, and learning; planning meals and shopping; organizing leisure activities; accompanying consumers to medical appointments and community activities; monitoring self-administration or administration of medications if AMAP; providing education and supportive services; documenting observations; addressing inappropriate behaviors per treatment plan; ensuring safety and handling emergencies; utilizing communication and de-escalation skills; contributing to crisis intervention as trained; transporting consumers in Prestera vehicles or personal vehicles; adhering to confidentiality, HIPAA, and risk-management policies; using the EHR system; meeting CPR/First Aid and crisis intervention training requirements; and participating in related projects directed by leadership.
