Job Summary
Direct Home Care Worker provides in-home and community-based care to frail elders, following the participant’s Care Plan. Responsibilities include personal care, assistance with activities of daily living, meal preparation/feeding, toileting/incontinence management, shopping, medication reminders, laundry and homemaking, transportation to appointments, and documentation of care on every visit. The role supports participants and families toward maximum self-reliance within the home, day health center, and dementia unit, and requires timely reporting of progress to the RNCM or clinic nurse. Must have a minimum High School Diploma or GED, at least 2 years of relevant experience, completion of a formal Direct Care/Nurse Assistant/Medical Assistant training program, annual infection control and safety trainings (including TB testing and BLS), ability to work with diverse populations, and maintain reliable transportation with a valid driver's license. Working conditions include driving within the PACE SEMI catchment area and exposure to infectious diseases and cleaning/medication hazards; the organization is noted as a Top Workplace in the area.
Required Qualifications
- Minimum High School Diploma or G.E.D.
- Must have minimum of (2) years of relevant work experience.
- Must have completed formal training program (Direct Care, Nurse Assistant, Medical Assistant, etc.) outside of PACE SEMI.
- Must participate in annual required in-service training and screening (infection control, TB testing, safety training, BLS).
- Must be medically cleared for communicable diseases and have immunizations up-to-date.
