Job Summary
Provide personal care and related services in the home under supervision, following the participant's plan of care to maintain hygiene and a safe environment. Prepare nutritious meals, assist with ambulation and therapy processes, and encourage independence through activities of interest. Give emotional support, establish trust with the participant and family, and report changes in physical or mental condition to staffing or nursing personnel. Perform routine housekeeping tasks and attend required in-services. Confirm weekly visit scheduling to coordinate necessary staff visits. The role requires the ability to drive or access homes that may not be wheelchair accessible, along with sufficient hearing, eyesight, and physical dexterity to perform assessments and lift up to 30 pounds. Candidates must hold a high school diploma, complete specific training or certification, pass a criminal background check, and demonstrate the ability to comprehend personal care and meal preparation basics while respecting ethics and confidentiality.
Required Qualifications
- High School Diploma or equivalent
- Successful completion of one of the following: a.) Minimum training meeting the requirement of HLTL, DOH, and CHHC b.) Training program developed by the agency which meets the criteria set forth in PA Code 611.55 Chapter 52 c.) Nurse aid certification and training program sponsored by the PA Department of Education
- If a nurse aide is certified by the state, the individual will meet the qualifications for a Personal Care Field Clinician as the hours of training will exceed the minimum required except for Orientation and Clinical Performance
- Must be free from health problems that may be injurious to participant/consumer, self and co-workers
- Applicant must present appropriate evidence to substantiate this
- Must comprehend the basics of personal care, housekeeping and meal preparation
- Must understand and respect participant/consumer including ethics and confidentiality of care
- Must have a complete criminal background check
- The ability to drive and/or the ability to access consumer's homes which may not be routinely wheelchair accessible
- Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the participant/consumer's condition and to perform participant/consumer care
- On occasion may be required to bend, stoop, reach and move participant/consumer weight up to 250 pounds: lift or carry up to 30 pounds
