Job Summary
Provide personal care and related services in the home under the direction of staffing and nursing personnel, following the participant's plan of care to maintain hygiene and a safe environment. Prepare nutritious meals, assist with ambulation and therapy processes, and encourage independence through activities of interest while offering emotional support. Report changes in mental or physical condition to supervisors and perform routine housekeeping tasks. Confirm weekly visit scheduling to coordinate necessary staff visits. Must be able to drive or access homes not routinely wheelchair accessible, with hearing, eyesight, and physical dexterity sufficient to lift up to 30 pounds. Requires a high school diploma, specific training certification (HLTL, DOH, CHHC, or state nurse aid), and a complete criminal background check. Must be free from health problems injurious to the consumer, self, or co-workers.
Required Qualifications
- High School Diploma or equivalent
- Successful completion of one of the following: a.) Minimum training meeting the requirement of HLTL, DOH, and CHHC b.) Training program developed by the agency which meets the criteria set forth in PA Code 611.55 Chapter 52 c.) Nurse aid certification and training program sponsored by the PA Department of Education
- Must be free from health problems that may be injurious to participant/consumer, self and co-workers
- Applicant must present appropriate evidence to substantiate this
- Must comprehend the basics of personal care, housekeeping and meal preparation
- Must understand and respect participant/consumer including ethics and confidentiality of care
- Must have a complete criminal background check
- The ability to drive and/or the ability to access consumer's homes which may not be routinely wheelchair accessible
- Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the participant/consumer's condition and to perform participant/consumer care
- Must be able to bend, stoop, reach and move participant/consumer weight up to 250 pounds
- Must be able to lift or carry up to 30 pounds
