Job Summary
Provide personal care and related services in the home under the direction of staffing and nursing personnel, following the plan of care to ensure safe and competent support. Assist participants with maintaining personal hygiene, ambulation, and therapy processes while encouraging independence through activities of interest. Plan and prepare nutritious meals, perform routine housekeeping tasks, and deliver simple emotional and psychological support to the participant, family, and household members. Report any changes in the participant's mental or physical condition or home situation to relevant personnel immediately. This role requires the ability to drive or access homes that may not be wheelchair accessible, along with sufficient hearing, eyesight, and physical dexterity for assessments and lifting up to 30 pounds. Applicants must possess a high school diploma, complete specific training or certification, maintain a clean criminal background check, and attend required in-services.
Required Qualifications
- High School Diploma or equivalent
- Successful completion of one of the following: a.) Minimum training meeting the requirement of HLTL, DOH, and CHHC b.) Training program developed by the agency which meets the criteria set forth in PA Code 611.55 Chapter 52 c.) Nurse aid certification and training program sponsored by the PA Department of Education
- Nurse aid certification and training program sponsored by the PA Department of Education
- Free from health problems that may be injurious to participant/consumer, self and co-workers
- Complete criminal background check
- Ability to comprehend the basics of personal care, housekeeping and meal preparation
- Ability to understand and respect participant/consumer including ethics and confidentiality of care
- Ability to drive and/or the ability to access consumer's homes which may not be routinely wheelchair accessible
- Sufficient hearing, eyesight and physical dexterity to perform a physical assessment of the participant/consumer's condition and to perform participant/consumer care
- Ability to bend, stoop, reach and move participant/consumer weight up to 250 pounds
- Ability to lift or carry up to 30 pounds
