Job Summary
Provide personal care and related services in the home under the direction of staffing and nursing personnel, following the plan of care to ensure safe and competent service. Maintain good personal hygiene, prepare nutritious meals, and assist with ambulation and therapy processes as needed. Encourage independence through activities of interest while offering emotional support and establishing trust with the participant, family, and household. Report changes in mental or physical condition or home situations to supervisors immediately. Perform routine housekeeping tasks and attend required in-services. Must be able to drive or access homes not routinely wheelchair accessible. Physical dexterity and senses must allow for assessments; lifting and carrying up to 30 pounds is required. High School Diploma or equivalent with specific training certification (HLTL, DOH, CHHC, or state nurse aide) is mandatory. Complete a criminal background check and demonstrate understanding of ethics and confidentiality.
Required Qualifications
- High School Diploma or equivalent
- Successful completion of one of the following: a.) Minimum training meeting the requirement of HLTL, DOH, and CHHC b.) Training program developed by the agency which meets the criteria set forth in PA Code 611.55 Chapter 52 c.) Nurse aid certification and training program sponsored by the PA Department of Education
- Must be free from health problems that may be injurious to participant/consumer, self and co-workers
- Applicant must present appropriate evidence to substantiate this
- Must comprehend the basics of personal care, housekeeping and meal preparation
- Must understand and respect participant/consumer including ethics and confidentiality of care
- Must have a complete criminal background check
- The ability to drive and/or the ability to access consumer's homes which may not be routinely wheelchair accessible
- Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the participant/consumer's condition and to perform participant/consumer care
- Must be able to bend, stoop, reach and move participant/consumer weight up to 250 pounds
- Must be able to lift or carry up to 30 pounds
