Job Summary
Direct Care Worker responsible for assisting patients with personal care activities, hygiene, mobility, and activities of daily living per the care plan; ensures a safe home environment, performs or assists with light housekeeping and meal preparation, monitors hydration and nutrition, and reports findings to the RN or clinical supervisor. Maintains confidentiality, follows infection control protocols, adheres to agency policies, completes required in-services, and supports team-based care within a home health setting. Positions available full-time, part-time, and temporary in Bensalem, PA.
Required Qualifications
- Direct Care Worker experience preferred
- Ability to assist with personal care
- Knowledge of infection control practices
- Ability to monitor patient health and report changes to RN
- Compliance with HIPAA and agency policies
