Direct Care RN

Conduct thorough patient assessments, design personalized care plans, and administer medications while monitoring vital signs and responding to changes in patient conditions. Coordinate care with an interdisciplinary team to ensure continuity, educate patients and families on health conditions and post-discharge care, and document all interventions according to regulatory standards. Perform daily rounding checks, admit patients, and communicate with family members to support overall patient well-being. Full-time, 40-hour week from 8 a.m. to 4:30 p.m. Monday through Friday. Requires current Virginia RN license and BLS certification within 90 days.
