Conduct thorough patient assessments, design personalized care plans, and administer medications while monitoring vital signs and responding promptly to clinical changes. Coordinate patient care with an interdisciplinary team to ensure continuity, educate patients and families on health conditions and post-discharge management, and document all interventions per regulatory standards. Requires current Virginia RN license and BLS certification within 90 days.
