Job Summary
Assist clients with daily living activities including meal preparation, hygiene care, and household organization. Provide transportation to appointments and errands while implementing individualized care plans and behavioral support strategies. Encourage positive behaviors, promote client dignity and community involvement, and ensure safety following agency protocols. Complete accurate documentation and progress notes. Respond to behavioral or medical emergencies as needed. Maintain professional boundaries and communicate effectively with families and agency leadership.
Required Qualifications
- High School Diploma or GED
- Valid driver's license and reliable personal vehicle for professional use
- Ability to pass a Level II Background Screening
- Experience supporting individuals with disabilities or behavioral health needs
- Demonstrated ability to manage client care and safety with limited supervision
- Personal smartphone required for app-based scheduling and client communication
- CPR/First Aid Certification
- APD Training Certificates
Desired Qualifications
- CNA, BLS, HHA, or DSP certifications
