Job Summary
Provide hands-on support for daily living activities including cooking, cleaning, hygiene, dressing, and mobility. Assist with personal care such as bathing, toileting, and medication reminders while supporting individuals in developing independent skills like budgeting and meal planning. Offer transportation to medical appointments and community outings, and implement behavioral support strategies to encourage participation in social and recreational activities. Collaborate with clinical team members to assist with goal planning and maintain accurate documentation of services and progress.
Required Qualifications
- High school diploma or equivalent
- Valid driver's license and reliable transportation

- 1-2 years experience preferred
