Job Summary
Conduct daily direct care for members with behavior disorders by implementing personalized treatment plans with case managers. Teach essential life skills including housekeeping, hygiene, and meal preparation to foster independence. De-escalate conflict situations and guide members toward positive solutions while serving as a role model. Assist with meals, administer medication, and monitor health conditions, reporting changes promptly. Document all services provided and complete required paperwork effectively. Maintain accurate records and collaborate with colleagues to support goal achievement.
