Job Summary
Hands-on residential care role providing daily living support, personal care, and mobility assistance to individuals with developmental disabilities in a home and community setting. Duties include coordinating and attending medical appointments, implementing individualized behavioral and support plans, administering medications per procedures, maintaining a clean and safe living environment, providing transportation as needed, completing documentation and progress reports, and collaborating with families, care providers, and team members to help residents build independence and participate in community activities.
Required Qualifications
- High School Diploma or equivalent
- Valid Driver’s License
- Successful completion and maintenance of all state and agency mandated trainings, including medication administration
- Must be willing to authorize a Criminal History Background Check (CORI) and release of information concerning allegations from DPPC, DDS, and other agencies
- Must be able to perform each essential duty satisfactorily
