Job Summary
Coordinate care processes in close collaboration with nursing relatives acting as central partners in care delivery. Create individual care plans considering client needs and provide on-site and telephone guidance to nursing relatives. Work closely with relatives, doctors, and other health service providers to ensure optimal care quality. Document care services, monitor performance, and continuously adjust care planning based on current developments. Requires a Diploma in Nursing (HF/FH) or equivalent, experience in home care, empathy, and a driver's license. Flexible 40-hour week with regulated hours, no weekend or night shifts, and home office options available.
Required Qualifications
- Abgeschlossene Ausbildung als Dipl. Pflegefachperson HF/FH oder eine gleichwertige SRK Anerkennung
- Führerausweis Kategorie B
- eigenes Auto
- Bereitschaft, im Raum Solothurn & Aargau mobil zu sein
Desired Qualifications
- Berufserfahrung im Bereich der Spitex, Psychiatrischen- und/ oder Kinderspitex
