Digital Health Specialist - Physical Therapist - Hybrid-Full-Time 2 Days on-siteand 3 Days Teleheal

Conduct virtual care and remote therapeutic monitoring services while delivering personalized plans of care for diverse orthopedic and neurologic patient populations. Objectively measure patient outcomes using cutting-edge software and efficiently document evaluations, treatments, and discharge notes. Perform telehealth visits including assessment and treatment plan development, then communicate with primary treating clinicians to revise care as appropriate. Serve as a liaison between patients and in-clinic providers, maintain regional service metrics reporting, and participate in digital health program development and marketing efforts. Requires a Physical Therapy state license and digital/telehealth experience.
