Job Summary
Hybrid Digital Health Specialist role combining in-clinic physical therapy with telehealth. Deliver digital health services including virtual care and remote therapeutic monitoring; develop and deliver a personalized plan of care for a diverse patient population with orthopedic and neurologic diagnoses; objectively measure patient outcomes using cutting-edge software; efficiently document evaluations, treatments, re-evaluations, and discharge notes; utilize a digital platform to deliver telehealth visits including evaluation, assessment, and treatment plan development; perform Telehealth and RTM services; collaborate with operations to scale remote services; communicate with the primary treating clinician to revise treatment as appropriate; serve as a liaison between providers and the Director of Digital Health to optimize and grow remote services; maintain regular reporting and data analysis of regional service metrics; participate in program development and marketing efforts as directed.
Required Qualifications
- Must be a board licensed Physical Therapist
- Diploma from a CAPTE-approved Physical Therapy Program
- Physical Therapy state license (FL)
- Digital/telehealth experience preferred!
