Digital Health Specialist - Physical Therapist - Hybrid

Job Summary
Develop personalized plans of care for diverse orthopedic and neurologic patients while objectively measuring outcomes using cutting-edge software. Deliver telehealth visits and perform Remote Therapeutic Monitoring (RTM) services, including interactive communication and treatment plan development. Efficiently document evaluations, treatments, re-evaluations, and discharge notes while serving as a liaison between patients and in-clinic providers. Collaborate with the Director of Digital Health to optimize remote services, participate in program development and marketing efforts, and maintain regional service metrics reporting. Work 3 days in clinic and 2 days remotely in a hybrid schedule.
Required Qualifications
- Must be located in Jacksonville, FL (Oakleaf) or within driving distance
- Diploma from a CAPTE-approved Physical Therapy Program
- Physical Therapy state license in Florida (or in process)
- Minimum 2 years of relevant clinical experience (Outpatient)
Desired Qualifications
- Experience providing digital health services preferred
