Job Summary
Hybrid full-time Digital Health Specialist/Physical Therapist providing telehealth and RTM services. Develop and deliver personalized care plans for a diverse outpatient population with orthopedic and neurologic diagnoses; document evaluations, treatments, re-evaluations, and discharge notes; measure outcomes using digital platforms; deliver telehealth visits and remote therapeutic monitoring; liaise between patients, in-clinic treating providers, and the Director of Digital Health to optimize and grow remote services; contribute to program development and marketing efforts; flexible to obtain licensure in other states.
Required Qualifications
- Must be a board licensed Physical Therapist to practice
- Diploma from a CAPTE-approved Physical Therapy Program
- Physical Therapy state license (FL)
- Must be willing to obtain licensure in other states
- Experience in the Outpatient setting preferred
