Job Summary
Hybrid Digital Health Specialist role combining in-clinic and telehealth Physical Therapy. Provide outpatient PT, develop and deliver personalized plans of care for a diverse patient population with orthopedic and neurologic diagnoses, and objectively measure patient outcomes using cutting-edge software. Efficiently document evaluations, treatments, re-evaluations, and discharge notes. Use digital platforms to deliver telehealth visits and remote therapeutic monitoring (RTM). Collaborate with operations to meet patient care and staffing needs; liaise between providers and the Director of Digital Health to optimize and grow remote services; maintain regular reporting and data analysis of regional service metrics; participate in program development and marketing efforts.
Required Qualifications
- Board licensed Physical Therapist
- Diploma from a CAPTE-approved Physical Therapy Program
- Physical Therapy state license (FL)
- Willing to obtain licensure in other states
- Outpatient setting experience
