Job Summary
Develop and deliver personalized plans of care for diverse patients with orthopedic and neurologic diagnoses while objectively measuring outcomes using cutting-edge software. Perform telehealth visits and Remote Therapeutic Monitoring (RTM) services, including interactive communication and assessment, via digital platforms. Efficiently document evaluations, treatments, and discharge notes, then communicate with primary treating clinicians to revise treatment plans as appropriate. Serve as a liaison between providers and the Director of Digital Health to optimize remote services, support operations, and maintain regional service metrics. Participate in program development and marketing efforts to grow digital health offerings. This hybrid role requires a Florida-licensed Physical Therapist with at least two years of outpatient experience, split between three days in the Jacksonville clinic and two days remote.
Required Qualifications
- Must be located in Jacksonville, FL ( Oakleaf) (or within driving distance)
- Diploma from a CAPTE-approved Physical Therapy Program
- Physical Therapy state license in Florida (or in process)
- Minimum 2 years of relevant clinical experience (Outpatient)
Desired Qualifications
- Experience providing digital health services preferred
