Job Summary
Hybrid Digital Health Specialist/Physical Therapist in Palm Coast, FL delivering in-clinic care three days a week and telehealth/remote therapeutic monitoring two days a week. Responsible for developing and delivering personalized plans of care for a diverse patient population with orthopedic and neurologic diagnoses, objectively measuring patient outcomes using cutting-edge software, documenting evaluations, treatments, re-evaluations, and discharge notes, and delivering telehealth and RTM services. Serves as a liaison between patients, in-clinic treating providers, and the Director of Digital Health to optimize and grow remote services; participates in program development, marketing efforts, and regional service metrics reporting. Requires CAPTE-approved PT training and Florida licensure (or in process) with a minimum of 2 years outpatient experience; digital health experience is preferred.
Required Qualifications
- Diploma from a CAPTE-approved Physical Therapy Program
- Physical Therapy state license in Florida (or in process)
- Minimum 2 years of relevant outpatient clinical experience
- Experience providing digital health services preferred
