Job Summary
Hybrid Digital Health Specialist role for a licensed Physical Therapist in outpatient care. The DHS provides digital health services including virtual care and remote therapeutic monitoring, develops and delivers a personalized plan of care for diverse orthopedic and neurologic patients, measures outcomes with digital tools, documents evaluations and treatments, conducts Telehealth and RTM visits, and collaborates with in-clinic providers and the Director of Digital Health to expand remote services. Responsibilities include liaising between patients and treating clinicians, supporting operations to deliver digital services, reporting regional metrics, and contributing to program development and marketing efforts. Requires a CAPTE-approved PT program credential and a FL license; telehealth/digital health experience preferred; willing to obtain licensure in other states.
Required Qualifications
- Board licensed Physical Therapist
- Diploma from a CAPTE-approved Physical Therapy Program
- Physical Therapy state license (FL)
- Experience in Outpatient setting preferred
