Digital Health Specialist - Physical Therapist - Hybrid

Provide hybrid physical therapy services as a Digital Health Specialist, delivering telehealth and remote therapeutic monitoring (RTM) to a diverse outpatient population. Develop and deliver personalized plans of care, objectively measure patient outcomes using cutting-edge software, efficiently document evaluations, treatments, re-evaluations, and discharge notes, and use a digital platform to conduct telehealth visits including evaluation, assessment, and treatment plan development. Serve as a liaison between patients and in-clinic treating providers and collaborate with the Director of Digital Health to optimize and grow remote services. Participate in program development, marketing efforts, and regional data reporting to support CORA’s digital health program. Must be a board-licensed Physical Therapist in Florida with CAPTE-approved program diploma; Digital/telehealth experience preferred; full-time, 40 hours/week; hybrid role with 3 days in clinic and 2 days remote; must be willing to obtain licensure in other states.
