Job Summary
Digital Health Specialist role for a Physical Therapist in a hybrid setting in Orange Park, FL. Responsibilities include delivering digital health services, including telehealth and remote therapeutic monitoring (RTM), developing the digital health program, and acting as a liaison between patients and in-clinic treating providers. The position supports in-clinic care 3 days/week and remote work 2 days/week, with a focus on delivering outpatient physical therapy services, measuring patient outcomes with software, documenting evaluations and treatment plans, and collaborating with the Director of Digital Health to optimize and grow remote services.
Required Qualifications
- Diploma from a CAPTE-approved Physical Therapy Program
- Florida Physical Therapy license (or in process)
- Minimum 2 years of relevant outpatient clinical experience
- Experience providing digital health services preferred
