Job Summary
Hybrid Digital Health Specialist role combining in-clinic physical therapy (3 days/week) with telehealth/remote therapeutic monitoring (2 days/week). Responsibilities include delivering digital health services, virtual care, and serving as a liaison between patients and in-clinic treating providers; develop and deliver personalized plans of care for a diverse patient population; objectively measure patient outcomes using software; efficiently document evaluations, treatments, re-evaluations, and discharge notes; conduct Telehealth and RTM services; support operations to deliver digital services and meet staffing needs; collaborate with the Director of Digital Health to optimize and grow remote services; maintain regular reporting and data analysis of regional service metrics; participate in program development and marketing efforts. Must be located in Rincon, GA (or within driving distance).
Required Qualifications
- Diploma from a CAPTE-approved Physical Therapy Program
- Physical Therapy state license in GA (or in process)
- Minimum 2 years of relevant outpatient clinical experience
- Experience providing digital health services preferred
