Job Summary
Hybrid Physical Therapist and Digital Health Specialist delivering in-clinic and telehealth care. Develop and deliver personalized plans of care for a diverse outpatient population with orthopedic and neurologic diagnoses; document evaluations, treatments, re-evaluations, and discharge notes; measure patient outcomes using digital platforms; conduct Telehealth visits and Remote Therapeutic Monitoring; collaborate with operations and treating clinicians to scale remote services; serve as a liaison between providers and the Director of Digital Health to optimize digital health programs and contribute to marketing and metric reporting.
Required Qualifications
- Board licensed Physical Therapist
- License in Florida (FL) or ability to obtain licensure in other states
- CAPTE-approved Physical Therapy Program diploma
