Digital Health Specialist - Physical Therapist - Hybrid

Develop and deliver personalized plans of care for diverse patients with orthopedic and neurologic diagnoses using digital platforms. Perform telehealth visits and Remote Therapeutic Monitoring services, including interactive communication and patient outcome measurement. Efficiently document evaluations, treatments, and discharge notes while maintaining regular reporting of regional service metrics. Serve as a liaison between patients and in-clinic providers, communicating with primary clinicians to revise treatment plans as appropriate. Participate in digital health program development and marketing efforts to optimize remote services. Work 3 days in person at the CORA clinic and 2 days remotely in a hybrid schedule.
