Job Summary
Hybrid Digital Health Specialist/Physical Therapist role delivering outpatient PT with integrated digital health services. You will provide in-clinic orthopedic/neurologic care three days per week and remote telehealth/RTM services two days per week, develop and deliver personalized care plans, objectively measure patient outcomes with digital tools, document evaluations and treatments, and liaise with in-clinic providers and the Director of Digital Health to scale remote services. Responsibilities include conducting telehealth visits, performing RTM services, supporting operations to deliver digital services, and contributing to program development and regional health metrics.
Required Qualifications
- Board licensed Physical Therapist
- Physical Therapy license (FL)
- Diploma from a CAPTE-approved Physical Therapy Program
