Job Summary
Hybrid Digital Health Specialist role for a Licensed Physical Therapist in outpatient setting; combines in-clinic 3 days/week with remote telehealth and RTM duties. The DHS provides digital health services including virtual care and remote therapeutic monitoring, develops the digital health program, and liaises between patients and in-clinic treating providers. Responsibilities include delivering telehealth visits, documenting evaluations and treatment plans, measuring patient outcomes, coordinating with the Director of Digital Health to grow remote services, and participating in program development and marketing. Location: Syracuse, NY (hybrid: 3 days in-person, 2 days remote).
Required Qualifications
- Board licensed Physical Therapist
- CAPTE-approved Physical Therapy Program diploma
- Physical Therapy state license (FL)
- Experience in outpatient setting
- Digital/telehealth experience preferred
