Digital Health Specialist - Physical Therapist - Hybrid

Hybrid Digital Health Specialist and Physical Therapist role delivering in-person rehabilitation at a CORA clinic part of the week with remote, telehealth-enabled care the remaining days. Responsibilities include developing and delivering personalized plans of care for diverse orthopedic/neurologic patient populations, delivering telehealth visits and Remote Therapeutic Monitoring (RTM), objectively measuring patient outcomes with digital tools, documenting evaluations, treatments, re-evaluations, and discharge notes, and acting as a liaison between patients, in-clinic treating providers, and the Director of Digital Health. The role supports operations to deliver digital services, contributes to program development and marketing efforts, and participates in data reporting and regional service metrics. Requires licensure in Florida and willingness to obtain licensure in other states; promotes equal opportunity in employment.
