Digital Health Specialist - Physical Therapist - Hybrid

Job Summary
Develop personalized plans of care for diverse patients with orthopedic and neurologic diagnoses while objectively measuring outcomes using cutting-edge software. Deliver telehealth visits and perform Remote Therapeutic Monitoring services, including interactive communication and treatment plan development via digital platforms. Efficiently document evaluations, treatments, and discharge notes, then communicate with primary treating clinicians to revise care as needed. Serve as a liaison between patients, providers, and the Director of Digital Health to optimize remote services and participate in program development and marketing efforts. Maintain regular reporting and data analysis of regional service metrics. Work 40 hours weekly in a hybrid model with three clinic days and two remote days at the South Flagler location.
Required Qualifications
- Must be located in Palm Coast, FL (or within driving distance)
- Diploma from a CAPTE-approved Physical Therapy Program
- Physical Therapy state license in Florida (or in process)
- Minimum 2 years of relevant clinical experience (Outpatient)
- Experience providing digital health services preferred
