Job Summary
Hybrid Digital Health Specialist and Physical Therapist role in Winter Haven, FL. Deliver telehealth and remote therapeutic monitoring as part of a digital health program; develop and deliver personalized plans of care for a diverse patient population with orthopedic and neurologic diagnoses; objectively measure patient outcomes using cutting-edge software; efficiently document evaluations, treatments, re-evaluations, and discharge notes; use a digital platform to conduct telehealth visits and RTM services; collaborate with in-clinic treating providers and serves as a liaison between providers and the Director of Digital Health to optimize and grow remote services; participate in program development and marketing efforts as directed by the Director of Digital Health.
Required Qualifications
- Must be a board licensed Physical Therapist to practice
- Diploma from a CAPTE-approved Physical Therapy Program
- Physical Therapy state license (FL)
- Digital/telehealth experience preferred
- Must be willing to obtain licensure in other states
