Job Summary
Hybrid role delivering Digital Health Physical Therapy services, including telehealth and remote therapeutic monitoring, and serving as a liaison between patients and in-clinic providers. Develop and deliver personalized plans of care for a diverse patient population with orthopedic and neurologic diagnoses; objectively measure patient outcomes using digital software; document evaluations, treatments, re-evaluations, and discharge notes; conduct telehealth visits and RTM services; support operations to deliver digital services and meet patient care needs; communicate with treating clinicians for treatment revisions; contribute to program development and marketing under the Director of Digital Health; participate in regional data reporting and professional development opportunities.
Required Qualifications
- Diploma from a CAPTE-approved Physical Therapy Program
- Physical Therapy state license (or in process)
